You will need a domain name and a hosting account (if you don't have them, I will help you). We will also need your logo (optional), the name and number of pages, a few sentences about your website/company, the e-mail address to set up the form, a few images from Google.

No, we do not provide hosting or domain name registration. In order to have full control over the website, we strongly recommend that you purchase them in your own name.
Please let us know which provider you purchased your domain from, and we can guide you through the process. Most providers can help you set this up fairly easily. But we can assist you with this.
